Emergency Information & Enrollment Packet
Attention new families! It’s time to enroll for 2020-2021. All contracts, except children enrolling in 4k programs, must be accompanied by a non-refundable registration fee. Please remember that 4k families must also register with the school district. In order to get a contract, please stop by our office or call us with your mailing address so we can send one directly to you.
If you are enrolling your child in our preschool, please download and complete the enrollment packet and turn into the LRP office prior to the day your child begins the program.

Below is the emergency form we require when you enroll your child in our program. This provides a digital record of your child’s parent/guardian information, as well as record of who is authorized to pick up your child from LRP. Once you have already filled out this form and you desire to make any changes to the information, please contact the office and we will modify the record for you.
